Each state is not required to participate in Medicaid, although every state currently does and complies with federal Medicaid laws. Each state sets standards of eligibility, how much is paid into it, the types of services covered, and all of these changes from state to state as each state administers its own program. In the year 2002, there were close to 40 million Americans enrolled in the program, with the majority of them being children. By the year 2009, there were close to 63 million Americans enrolled in Medicaid and receiving different services and coverage.
The Diagnostic and Statistical Manual of Mental Disorders (DSM) once referred to substance abuse and substance dependence as diagnostic terms. However, in the updated fifth edition (DSM-5), these terms are replaced by the singular substance use disorder, which is broken into mild, moderate and severe to refer to the physical and mental impairments through recurrent substance use.
According to the National Institute on Drug Abuse, addiction is a "chronic, relapsing brain disease that is characterized by compulsive drug seeking and use, despite harmful consequences."7  There is no simple cure for addiction; however, effective treatment can help you become and stay sober.7  You will have to manage your addiction throughout your life, the same way a diabetic has to manage their condition with ongoing efforts like a proper diet and exercise.8
The physician must state firmly, but empathically, that alcohol is a problem for the patient and that the patient determines the solution. Patients come for treatment through several means, often from a mixture of both coercion and concern. The clinician needs to understand the extent of resistance to effectively work with the patient. A good strategy is to learn about patients' goals and indicate discrepancies between their goals and their choices. Pointing out discrepancies is more effective initially than statements such as, "You have to quit," or, "You have to go to AA."
Opioids: Methadone (Dolophine®, Methadose®), buprenorphine (Suboxone®, Subutex®, Probuphine® , Sublocade™), and naltrexone (Vivitrol®) are used to treat opioid addiction. Acting on the same targets in the brain as heroin and morphine, methadone and buprenorphine suppress withdrawal symptoms and relieve cravings. Naltrexone blocks the effects of opioids at their receptor sites in the brain and should be used only in patients who have already been detoxified. All medications help patients reduce drug seeking and related criminal behavior and help them become more open to behavioral treatments. A NIDA study found that once treatment is initiated, both a buprenorphine/naloxone combination and an extended release naltrexone formulation are similarly effective in treating opioid addiction. Because full detoxification is necessary for treatment with naloxone, initiating treatment among active users was difficult, but once detoxification was complete, both medications had similar effectiveness.

A growing literature is demonstrating the importance of emotion regulation in the treatment of substance abuse. Considering that nicotine and other psychoactive substances such as cocaine activate similar psycho-pharmacological pathways,[44] an emotion regulation approach may be applicable to a wide array of substance abuse. Proposed models of affect-driven tobacco use have focused on negative reinforcement as the primary driving force for addiction; according to such theories, tobacco is used because it helps one escape from the undesirable effects of nicotine withdrawal or other negative moods.[45] Acceptance and commitment therapy (ACT), is showing evidence that it is effective in treating substance abuse, including the treatment of poly-substance abuse and cigarette smoking.[46][47] Mindfulness programs that encourage patients to be aware of their own experiences in the present moment and of emotions that arise from thoughts, appear to prevent impulsive/compulsive responses.[45][48] Research also indicates that mindfulness programs can reduce the consumption of substances such as alcohol, cocaine, amphetamines, marijuana, cigarettes and opiates.[48][49][50]


For example, someone with bipolar disorder that suffers from alcoholism would have dual diagnosis (manic depression + alcoholism). In such occasions, two treatment plans are needed with the mental health disorder requiring treatment first. According to the National Survey on Drug Use and Health (NSDUH), 45 percent of people with addiction have a co-occurring mental health disorder.
A large body of scientific evidence has been gathered in recent years to show that addiction can run in families. In fact, children of alcohol-addicted parents are four times more likely to develop alcohol addiction in later life than those born to parents without alcohol addictions. How this works is complex, and there is no one ‘alcohol gene’ to blame for this; instead a number of genetic variations, which mean some individuals are more pre-disposed to alcoholism than others.
Focus on one area where you’re experiencing the urge. How do the sensations in that area feel. For example, perhaps you feel hot, cold, tingly, or numb? Are your muscles tense or relaxed? How large an area is involved? Describe the sensations to yourself and any changes that occur. “My mouth feels dry and parched. There is tension in my lips and tongue. I keep swallowing. As I exhale, I can imagine the smell and tingle of a drink.”
